Sheffield CCG Health Fund

Administered by South Yorkshire Community Foundation Minimum grant size is £5,000 and maximum grant size is £50,000 The deadline for applications is Monday 23rd February 2015   Criteria: The fund will be distributed to eligible community and voluntary groups, charities and charitable organisations where the grant will make a significant difference and assist with the long term viability of sustainable health support of the residents of Sheffield.

This pot of £550,000 will be delivered in 2 strands: Just under 20%  will be designated for mental health priorities, and applications will be encouraged for projects that contribute to at least one of the following criteria: Waiting list reduction initiatives Reductions in the number of people requiring emergency care due to a mental health crisis Support for carers of people with mental illness Providers may offer short- term services that will generate more capacity within the Sheffield Health community and/ or offer preventative early intervention services for service users and carers Size of Grants: £5,000 to £25,000 Just over 80% will be designated for health priorities in line with the Sheffield CCG main objectives for 2014-19, and applications are sought for projects that contribute to at least one of the following criteria: Reducing urgent hospital admissions. Support people with long- term conditions to keep well and remain living at home. Move provision of care to a community setting. Reducing life expectancy inequalities in adults with serious mental illness or with learning disabilities. Help all children have the best possible start in life. Applications from organisations preparing to work with Sheffield City Council as part of the -Keeping People Well in their Communities initiative, to support their development and preparation for that work, are welcome. Size of Grants: £10,000 to £50,000 CCG are keen to ensure that applications contribute to its aims over the longer term and that activities are sustainable Who can apply? Not for profit charities or charitable organisations / constituted community groups and voluntary organisations or associations with a social purpose and with at least 3 unrelated people aged over 18 on the management committee where the beneficiaries are residents of Sheffield. Examples of the type of activities that will be funded Capital and / or revenue costs Running costs including salaries, core costs and project costs, committee and volunteer / staff training Exclusions Grants will not be awarded to: individuals; statutory organisations, including schools; fund trips abroad; fund buses, mini buses or other community transport schemes (not including transport costs forming part of a project); Bids should not aim to duplicate any element of the Ageing Better programme You can now apply online via our website http://www.sycf.org.uk/apply_for_a_grant/sheffied_ccg_health_fund/ to complete Part A of the form and download Part B and guidance notes.
